- Birth Certificate
- Social Security Card (if applicable)
- Immunization Card or Record
- Report Card from the Previous School
- Withdrawal Form ( if enrollment occurs during the school year)
- Proof of Residence (current water or electric bill)
- Valid Photo ID of the Parent/Guardian
- If transferring from Mexico, please provide a conduct letter from the previous school
NOTE: Only Parents or Legal Guardians can register a student.
**If a student is not living with the parents, a notarized guardianship letter is required in order to enroll.
***If the name on the utility bill does not match the person that is registering, notarized proof of residence form is required.
